AI VOICE AGENTS BY ROLE
AI voice agents for appointment reminders
A reminder agent calls people before their appointment, confirms they're coming, and offers to reschedule or cancel if they can't. Unlike a text, it can answer a question ("Is parking free?"), take a new time request and tell you who won't come in time to fill the slot.

Why reminders matter
- A review of 105 studies put the average healthcare no-show rate around 23%, with long lead times and a history of no-shows as the strongest predictors.
- NHS England estimated that around 15.4 million GP-practice appointments a year were missed without notice, costing more than £216 million and over 1.2 million GP hours.
What the research says about phone reminders
- In a Cochrane review of 8 trials (6,615 people), attendance was 67.8% with no reminder, 78.6% with a text and 80.3% with a phone call. Texts cost 55–65% less per attendance in the two studies that measured cost.
- A BMJ Open meta-analysis found reminded patients were 23% more likely to attend and 25% less likely to miss; voice notifications appeared more effective than texts, and several reminders worked better than one.
- Another review found reminders cut non-attendance by a third on average, with manual phone calls outperforming automated reminders (39% vs 29% reduction). Sending the day before or the week before made no clear difference.
This evidence is about human calls, texts and recorded messages; no trial has yet measured conversational AI reminder calls. The working idea is that an agent that can hold a conversation behaves more like a staff call than a recorded message, at the cost of a text.
What a good reminder call does
- Identifies the caller and the appointment — the business name first, then date and time.
- Asks one question: "Can you still make it?"
- Handles the answer: confirm; or capture a preferred new time and hand it to your booking system or a person; or record a cancellation so the slot can be offered again.
- Keeps it short and never adds marketing.
- Respects "don't call me" immediately.
The rules for reminder calls
This is a summary, not legal advice.
| Where | What applies |
|---|---|
| United States, healthcare | Providers may call a patient's mobile with appointment reminders without prior consent only under the TCPA healthcare exemption: the number the patient gave, the provider's name and contact details at the start, no marketing or billing, generally one minute or less, at most one call a day and three a week, and an easy opt-out. |
| United States, other informational calls | Non-marketing artificial-voice calls to a home line are limited to three in 30 days and must honour opt-outs; calls to mobiles with an AI voice need prior express consent. |
| European Union | Tell people they're speaking with an AI at the latest at the first interaction, from 2 August 2026, and handle recordings as personal data. |
How to run it in SAGOT
- Write the agent. The greeting names your business and the appointment; instructions say what it can change and who handles anything else.
- Set the calling policy. Calling hours and days in your time zone, attempts per contact (up to 5), automatic retries with growing gaps and a maximum call length. Every call speaks your AI and recording disclosure; audio recording can be turned off and transcripts kept.
- Run a campaign with the day's or week's appointments, and connect your booking system so confirmations and reschedule requests arrive as actions — or as handoffs for a person.
- Review results: who confirmed, who wants another time, who didn't answer, with a transcript for each.
SAGOT is in a closed alpha. New phone lines start in test mode, where only the line's test numbers can be called. Join the waitlist, and draft your script with the free call script generator.
Frequently asked questions
Do AI reminder calls reduce no-shows?
Reminders by phone and text clearly improve attendance in the research above. Trials of conversational AI calls specifically haven't been published yet, so measure your own attendance before and after.
Are phone reminders better than texts?
Phone calls did slightly better than texts in the Cochrane review and voice did better in the BMJ Open analysis, while texts were cheaper. A voice agent can also handle rescheduling in the same call.
When should a reminder call be made?
The review that compared timing found no clear difference between the day before and the week before. Many teams call two or three days ahead so freed slots can be refilled.
Do I need consent to make reminder calls?
It depends on the country and the line. In the US, healthcare reminders to mobiles fall under a narrow exemption with strict limits; other AI-voice calls to mobiles need prior consent.
